Each prospective client will need to present copies of the following:

  • Birth Certificate
  • Immunization Record
  • Physical Examination
  • Insurance Card
  • Social Security Card
  • School Records (report card, IEP ) for children and adolescents
  • Proof of Guardianship (for children)
  • Prior Mental Health History

PSYCHIATRIC REHABILITATION PROGRAM (PRP)

Psychiatric Rehabilitation Program (PRP) works to assist individuals living with mental illness to reach a higher level of independence. The PRP program assists participants with accessing and coordinating services and resources in their communities and works to ensure that each individual has access to the best possible treatment and care.

Our PRP services include but are not limited to:

Self Care

  • Personal Hygiene
  • Nutrition
  • Planning and Cooking Meals
  • Managing Medications
  • General Health Care Promotion
  • Personal Safety

Social Skills:

  • Interpersonal Skills
  • Stress and Anger Management
  • Communication
  • Socialization

Recreation 

  • Independent Living And Community Living Skills Training
  • Maintaining Housing
  • Utilizing Community Resources
  • Transportation and Mobility Training
  • Personal Finance and Budgeting
  • Opportunities to Develop and Practice Employment and Leadership Skills
  • Acquainting Clients with their Local Community

PRP ELIGIBILITY AND INTAKE PROCESS FOR CHILDREN & YOUTH

To be eligible for our program, youth must satisfy ALL of the following
requirements:

  • 6-17 years of age
  • Referral from treating Licensed Mental Health Professional
  • Have an Axis I diagnosis from a licensed mental health professional
  • Currently have ACTIVE Maryland State Medicaid
  • Intake and Rehabilitation Assessment with National Pike staff (Parent or Legal Guardian Must be Present)

PRP ELIGIBILITY AND INTAKE PROCESS FOR ADULTS

To be eligible for our program, adults must satisfy ALL of the following requirements:

  • Referral from treating Licensed Mental Health Professional
  • Maryland Medicaid and/or proof of application
  • A psychiatric diagnosis that meets the priority population definition as determined by the Mental Health Authority.
  • Intake and Rehabilitation Assessment with National Pike staff
